The initial step in your registration process begins with completing our recruit questionnaire. YOU MUST COMPLETE A RECRUIT QUESTIONNAIRE.

1. Scan & email the following documents (except official transcripts, they must be sent by high school/college, e-transcripts are accepted)
- Copy of your Valid State ID, Passport or Alien Registration Card
- Copy of your Social Security Card
- Copy of your Birth Certificate
- High School or GED diploma & sealed official high school transcript (HS Grads) - send to snatal@asa.edu
- Sealed official college transcript (College Transfers)
- Immunization Records (MMR)
- Signed copies of parent's tax forms from previous year (W2 forms if married)
- Copy of parent's valid state ID or Passport
- $25 Application Fee (Cash/Check/Money Order)
